5-15-15 JOB PURPOSE:
Provide case management services to individuals and families to address social determinants of health.
Assist clients in identifying and obtaining available benefits, social and community resources.
Support quality improvement projects to improve overall health outcomes for targeted population groups.
ESSENTIAL DUTIES AND
Responsibilities:
Meet with patients individually or in families, to assess situations, capabilities, and problems to determine what services are required to meet their needs.
Document patient encounters both in person and by phone in the electronic medical record.
Provide information and refer patients to agencies and community services for assistance such as:
basic needs including food and housing, insurance, medication assistance, transportation, employment.
Assist patients in connecting to services internally and externally by making phone calls with the patient if the patient needs additional support to ensure the patient gets connected to the needed service.
Case manager to assist patients with completing applications and other forms as needed.
Consult with case management supervisor as needed for guidance on complex social patient cases or direction on tasks to complete.
SUPPLEMENTAL FUNCTIONS:
Perform miscellaneous job-related duties as assigned.
Attend and participate in training and other professional development activity.
Participate in performance-related goal setting and achievement to meet personal and organizational goals and objectives.

Education:
Bachelor's degree preferred or equivalent combination of education, training, and experience.
Preferred licensure:
Certified Social Worker by the Mental Health Licensing Board in the state of Wyoming Other:

Occasionally may be required to attend visits to patient homes with a multidisciplinary team.
